Why did I receive a bill from ARDX Mark and Kambour Pathology?

Your physician has chosen Aurora Diagnostics Mark and Kambour Pathology to diagnose and interpret the specimen removed at your recent doctor's appointment. A report will be sent directly to your physician to assist him/her with your care. Your physician will not bill you for our laboratory services; instead ARDX Mark & Kambour Pathology will directly bill your insurance or you.

Who is responsible for paying for pathology services?

Ultimately, it is the patient's responsibility to pay for services.

Who bills my commercial insurance for pathology services rendered?


Aurora Diagnostics Mark and Kambour has contracts with many insurance companies and will process an insurance claim with your insurance carrier on your behalf. If we do not contract with your insurance provider, we can still file a claim on your behalf. However, there is no guarentee that your insurance provider will pay the full amount. The patient is responsible for any unpaid balances.

What about claims to Medicare?

Aurora Diagnostics Mark & Kambour will process a claim for Medicare on your behalf. If you have secondary insurance, we can also file a claim with the insurance provider.

What information should I have ready when I call?

Please have your insurance card or explanation of benefits (EOB) from you insurance company so our Billing Representative may assist you.

Laboratory Billing Information

Your physician has performed a procedure that requires the services of a pathologist. Mark & Kambour Pathology Associates is the laboratory your physician selected to provide the interpretation of your biopsy and/or specimen. A report will be sent directly to your physician to assist him/her with your care. Your physician will not bill you for our laboratory services; instead Mark & Kambour Pathology Associates will directly bill your insurance or you. INSURANCE Information

Mark & Kambour Pathology Associates accepts all insurances even though it is important to mention we are not a participating provider with all insurances.

Working with HMOs

We bill the HMO insurance provided by the patient, primary and secondary (if applicable). Many times,
the insurance company will pay a portion of the total charges for out-of-network services and then send the patient an
EOB (explanation of benefits) indicating that the patient is responsible for the difference between the total charges
and the insurance payment.
